145 ASSESSING MEDICINE LISTS: A QUALITATIVE STUDY WITH MULTIPLE STAKEHOLDERS

Abstract Background Multimorbidity and polypharmacy often result in numerous interactions with different Healthcare Professionals (HCPs) many transitions of care. Keeping up-to-date medications list people taking medicines and/or carers can reduce medication errors at care transitions. The HSE National Quality Improvement team is working on a national safety campaign which encourages to keep lists. However, there limited information about perceptions Patient-Held Medication Lists (PHML) clinical practice. aim this study was examine attitudes PHML among taking/managing multiple medicines, HCPs; how lists are used Methods Purposive sampling employed recruitment through relevant organisations, social media snowballing methods. Semi-structured telephone interviews were conducted 39 people; HCPs (N=21), patients/caregivers (N=18). Interviews transcribed thematically analysed behavioural frameworks - the Consolidated Framework for Implementation Research (CFIR) Theoretical Domains (TDF). Results Three core themes identified: Attitudes PHML; Function preferred features Barriers facilitators future use PHML. All participants thought keeping had benefits both (e.g. empowering, improving adherence). who found them useful particular situations emergencies). patient/caregiver groups expressed concerns their accuracy.
